Objetivo: Comparar in vivo la capacidad de formación ósea de dos tipos de biomateriales diseñados como sustitutivos óseos respecto a autoinjerto de cresta iliaca, uno basado en carbonatohidroxiapatita y otro en vidrio mesoporoso bioactivo. Material y método: Estudio experimental compuesto por 14 conejos de Nueva Zelanda hembras adultas donde se realizó un defecto crítico en hueso radio. La muestra fue dividida en cuatro grupos: defecto sin material, con autoinjerto de cresta iliaca, con soporte de carbonatohidroxiapatita y con soporte de vidrio mesoporoso bioactivo. Se realizaron estudios seriados de radiología simple a las 2, 4, 6 y 12 semanas y estudio de micro-TC a eutanasia a las 6 y 12 semanas. Resultados: En el estudio de radiología simple, el grupo de autoinjerto mostró las mayores puntuaciones de formación ósea (7,5 puntos). Ambos grupos de biomateriales presentaron formación ósea similar (5,3 y 6 puntos, respectivamente) y mayor al defecto sin material (4 puntos), pero siempre menor que el grupo de autoinjerto. Los resultados del estudio de micro-TC mostraron el mayor volumen de hueso en el área de estudio en el grupo de autoinjerto. Los grupos con sustitutivos óseos presentaron mayor volumen de hueso que el grupo sin material, pero siempre menor que en el grupo de autoinjerto. Conclusiones: Ambos soportes parecen favorecer la formación ósea pero no son capaces de reproducir las características del autoinjerto. Por sus diferentes características macroscópicas cada uno podría ser adecuado para un tipo diferente de defecto.(AU)

BACKGROUND: Trans-olecranon fracture dislocations are the least frequent complex elbow instability. Proper surgical treatment should be performed to avoid postoperative complications. METHODS: A retrospective design study was performed. Patients that suffered from this injury, treated at our center from 2010 to 2016 were included. Fifteen patients were analyzed. Functional results were measured using DASH, MEPS and VAS scores. Average time from injury to first surgical treatment was 4.87 days. Radial head fracture was present in seven cases and coronoid process in three patients. Most frequent complication was hardware disturbances in five patients. Mean follow up was 3.65 years. RESULTS: Mean range of motion (ROM) was evaluated 1 year postoperatively: 129° flexion, 6° flexion contracture, and less than 5° deficit of pronation/supination. Clinical and functional results are encouraging, DASH 36.38, MEPS 100 and VAS 0.46. CONCLUSION: Trans-olecranon fracture dislocations could obtain functional range of movement, pain relief and good functional outcomes with a standardized protocol of surgical fixation. It is important to achieve proper ulnar fixation, focusing in reestablishing dorsal angulation, and also to treat radial head and coronoid injuries properly, if present.

INTRODUCTION: The objective of this study is to assess the type, frequency and severity of complications after the implantation of the modular monopolar radial head prosthesis. MATERIAL AND METHODS: Forty-seven patients with 48 radial head prostheses implanted between 2009 and 2017 were reviewed retrospectively. Patients were evaluated clinical and radiographically for a mean follow-up of 43.55 months (range: 12-89). RESULTS: The same type of prosthesis was implanted in every patient (Ascension Modular Radial Head). The average score in the Mayo Elbow Performance Score was 88.29 ± 9.9 points. During the follow-up, three patients (6.25%) suffered from continuous pain. Twelve cases (25.5%) showed radiological oversizing, though only five were symptomatic. Heterotopic ossification was detected in twenty-seven cases (57.4%). Eleven patients (23.4%) developed postoperative stiffness. Nineteen cases (40.42%) showed periprosthetic osteolysis, from which seven were symptomatic. Thirteen patients (27%) developed surgery-related complications: three cases of infection, four cases of symptomatic loosening, two neurapraxies, one instability and three cases of oversizing with associated stiffness. Nine patients (18.75%) required reintervention. DISCUSSION: Our study obtains a 27% of overall complications, mostly related to oversizing and prosthetic loosening, and 19% of reinterventions. These results are similar to those presented in previous studies, with variations depending on the time of follow-up. Further research is also required to evaluate long-term results and the potential progression of the radiographic findings. CONCLUSION: Taken together, these data stress the need for improvement in both the surgical technique and the design of the implants.

MATERIAL Y MÉTODOS: Se revisaron retrospectivamente 47 pacientes con 48 prótesis de cabeza radial implantadas entre 2009 y 2017 durante una media de 43.55 meses (rango: 12-89). RESULTADOS: Se implantó el mismo tipo de prótesis en cada paciente (Ascension Modular Radial Head). La puntuación media obtenida en la clasificación Mayo Elbow Performance Score fue de 88.29 ± 9.9 puntos. Durante el seguimiento tres pacientes (6.25%) sufrieron dolor continuo. Doce casos (25.5%) mostraron sobredimensión radiológica, aunque sólo cinco fueron sintomáticos. Se detectó osificación heterotópica en 27 casos (57.4%), 11 pacientes (23.4%) desarrollaron rigidez postoperatoria, 19 casos (40.42%) mostraron osteólisis periprotésica, de los cuales siete fueron sintomáticos, 13 pacientes (27%) presentaron complicaciones: tres casos de infección, cuatro casos de aflojamiento sintomático, dos neuroapraxias, una inestabilidad y tres casos de sobredimensionamiento con rigidez asociada. Nueve pacientes (18.75%) fueron reintervenidos. DISCUSIÓN: Presentamos 27% de complicaciones globales, principalmente relacionadas con la sobredimensión y el aflojamiento protésico y 19% de reintervenciones. Estos resultados son similares a los descritos en estudios previos con variaciones en función del tiempo de seguimiento. Asimismo, se requieren nuevos estudios para evaluar los resultados a largo plazo y la posible progresión de los hallazgos radiográficos. CONCLUSIÓN: En conjunto, estos datos ponen de manifiesto la necesidad de mejoría tanto de la técnica quirúrgica como del diseño de los implantes.

Introducción: La tríada terrible de codo es una enfermedad asociada a altas tasas de complicaciones, por ello queremos estudiar los resultados y las complicaciones. Material y métodos: Se ha utilizado la base de datos de nuestro hospital durante 2005-2015, recogiendo características del paciente, de la fractura, de la cirugía y las complicaciones asociadas, así como resultados funcionales y rango de movilidad. Resultados: Se obtuvieron un total de 62 tríadas, de las cuales 27 (43%) eran mujeres y 35 (56%) eran hombres. A todos se les realizó un abordaje lateral de Kaplan y en aquellos que lo necesitaron se complementó con un abordaje medial para reparar el LCM. La cabeza radial se sintetizó en 14 (22%) casos, se colocó una prótesis en 45 (74%) de casos y otras actuaciones en 3 (5%) casos. La osteosíntesis de la apófisis coroides se realizó mediante arpón+/-sutura transósea en 41 (62%) pacientes y mediante síntesis en 12 (19%) casos. En 9 (14%) casos no se realizó ninguna actuación. En el 100% de los casos se reparó el LCL y en 9 (14%) hubo que reparar también el LCM. Un 22% de los pacientes precisaron FE por inestabilidad tras la técnica quirúrgica. Respecto a resultados, se obtuvo un rango de movilidad de entre 120°/-20° de flexoextensión y 98°/85° de pronosupinación. En cuanto a complicaciones, obtuvimos un total de 17 (27%). Conclusiones: Las tríadas de codo son lesiones osteoligamentosas complejas donde es necesario realizar una cirugía protocolizada, a pesar de lo cual, existen un 27% de complicaciones

BACKGROUND: Terrible triad of elbow is a complex lesion with a high rate of complications. Our goal is to analyse both clinical results and complications after performing protocolised surgery. MATERIAL AND METHODS: The database of our hospital was used during 2005-2015, collecting characteristics of the patient, the fracture, the surgery and the associated complications, as well as functional results and range of mobility. RESULTS: A total of 62 triads were obtained, of which 27 (43%) were women and 35 (56%) were men. All had a Kaplan lateral approach and those who needed it were complemented with a medial approach to repair the LCM. The radial head was synthesized in 14 (22%) cases, a prosthesis was placed in 45 (74%) cases and other actions in 3 (5%) cases. RESUMO

Las coaliciones carpianas son relativamente frecuentes entre huesos de la misma hilera del carpo. Son mucho más infrecuentes las coaliciones entre huesos de diferente hilera. Presentamos un caso de coalición intercarpiana entre trapecio y escafoides bilateral como hallazgo casual, confirmando su origen hereditario ya que uno de los progenitores presenta la misma coalición

Objetivo. Las fracturas de escafoides Herbet tipo B1 y B2 se pueden tratar tanto de manera ortopédica con inmovilización como de manera quirúrgica. El objetivo de este estudio es analizar los resultados clinicofuncionales y las complicaciones de las fracturas de escafoides tratadas con fijación percutánea mediante abordaje volar. Material y método. Estudio retrospectivo de 92 pacientes, tratados en nuestro centro, entre 2006 y 2016, mediante fijación percutánea volar. Seguimiento medio de 16 meses (rango 12-48). La clasificación de las fracturas se realizó según la clasificación de Herbert, incluyendo los tipos de fractura B1 y B2. El resto de los tipos fueron excluidos. Se excluyeron a los pacientes politraumatizados y los abordajes dorsales, las fracturas asociadas a fracturas de radio y a los pacientes sin seguimiento. Los resultados funcionales se evaluaron utilizando el cuestionario DASH. El criterio de consolidación se estableció con ausencia de dolor en el examen físico y presencia de consolidación en las proyecciones radiográficas. Resultados. El tiempo medio para la consolidación fue de 6,6 semanas (rango 5-11). A los 12 meses de seguimiento, el rango promedio de movimiento fue 65° de extensión (rango 58-75) y 75° de flexión (rango 72-80). Obtuvimos una media de 42 puntos según el cuestionario DASH. La complicación más frecuente fue la seudoartrosis en 4 casos (4,4%). El tiempo de cirugía medio fue de 25 min. Conclusiones. La fijación percutánea volar es una técnica simple y rápida para un cirujano especializado, caracterizada por bajas tasas de morbilidad y complicaciones, en comparación con otras técnicas. Permite acortar el tiempo de convalecencia y acelera la recuperación funcional del paciente, aparte de ser coste-efectiva (AU)

Volar percutaneous fixation is a simple and quick technique for a specialist surgeon, characterised by low morbidity and complication rates compared to ORIF and orthopaedic treatment, which accelerates the patient's functional recovery (AU)

Introducción. Los esguinces de tobillo son una de las patologías traumatológicas más frecuentes. A pesar de un tratamiento conservador adecuado, este fracasa en el 20-40% de los casos. Estos pacientes suelen desarrollar una inestabilidad crónica de tobillo. En el 75-80% de las ocasiones existe una rotura aislada del ligamento talofibular anterior. Material y método. Estudio retrospectivo observacional de 21 pacientes intervenidos quirúrgicamente mediante técnica de reparación anatómica artroscópica, por inestabilidad lateral crónica de tobillo, entre mayo de 2012 y enero de 2013. Se realizó un seguimiento medio de 29 meses (rango de 25-33). La distribución por sexos fue de 15 hombres y 6 mujeres; la edad media fue de 30,43 años (rango de 18-48). En 4 pacientes (19%), se diagnosticó una deformidad en varo del retropié asociada. En 15 pacientes (71,4%), se diagnosticaron lesiones intraarticulares: 6 pacientes presentaron lesiones osteocondrales (LOC) talares, 3 presentaban un pinzamiento posterior de tobillo y 6 pacientes presentaban distintas patologías de los tendones peroneos. Todas estas lesiones fueron abordadas quirúrgicamente en el mismo acto quirúrgico. Resultados. Se llevó a cabo una revisión clínico-funcional, utilizando la escala American Orthopaedic Foot and Ankle Society (AOFAS). La media preoperatoria fue de 66,12 puntos (rango de 60-71) y la postoperatoria se incrementó hasta una media de 96,95 puntos (rango de 90-100). Todos los pacientes pudieron retomar sus actividades deportivas previas en una media de 21,5 semanas (rango de 17-28). Encontramos complicaciones en 3 pacientes (14%). Conclusiones. La reparación anatómica del ligamento talofibular anterior mediante esta técnica presenta unos resultados clínico-funcionales excelentes, con una temprana reincorporación del paciente a sus actividades deportivas y un bajo índice de reintervenciones y complicaciones (AU)

Arthroscopic anatomical ligament repair technique has excellent clinical-functional results with a low percentage of complications, and enables patients to return to their previous sport activity within a short period of time (AU)

Objetivo: Se realiza una revisión de los primeros casos intervenidos en nuestro centro de inestabilidad glenohumeral recidivante de hombro por vía artroscópica. El objetivo del trabajo es analizar la influencia de la curva de aprendizaje en los resultados obtenidos. Material y métodos: Estudiamos 137 pacientes intervenidos en el Hospital 12 de Octubre de Madrid, entre Febrero de 1999 y Marzo de 2010; 101 pacientes cumplían los criterios de inclusión, y dichos pacientes fueron divididos en dos grupos por orden cronológico: los 50 primeros y los 50 segundos. No existieron diferencias estadísticamente significativas en cuanto a sexo, edad y lateralidad entre ambos grupos (p = 0.51, p = 0.15 y p = 0.23, respectivamente), siendo por tanto los grupos comparables. Comparamos entre ambos grupos los resultados clínicos, evaluados por el número de luxaciones, reintervenciones y complicaciones: migración de los implantes, artrosis y neuropatía del nervio axilar. Comparamos también los resultados funcionales valorados por las escalas de Constant y Rowe. Resultados: Se produjeron cuatro episodios de reluxación en el grupo 1 y seis episodios en el grupo 2. Respecto al número de reintervenciones, se realizaron tres en el grupo 1 y seis en el grupo 2. No existen diferencias estadísticamente significativas en el número de episodios de reluxación y reintervención (p = 50 y p = 0.48, respectivamente). En nuestra serie la curva de aprendizaje no parece asociarse con mayor número de complicaciones ni índice de reluxaciones.


Objective: We reviewed the first cases that underwent arthroscopic surgery at our center due to relapsing glenohumeral stability of the shoulder. The objective of this paper is to analyze the influence of the learning curve on the results obtained. Material and methods: We analyzed 137 patients who underwent surgery at Hospital 12 de Octubre in Madrid, Spain between February 1999 and March 2010. A total of 101 patients met the inclusion criteria, and these patients were divided into two groups using a chronological order, the first 50 patients and the second 50 patients. There were no statistically significant differences in sex, age and laterality between both groups (p = 0.51, p = 0.15 and p = 0.23, respectively), so the groups were comparable. We compared the following between both groups: clinical outcomes, number of dislocations, reoperations and complications, i.e., implant migration, arthrosis and axillary nerve neuropathy. We also compared the functional results, which were measured using the Constant and Rowe scales. Results: Four episodes of redislocation occurred in group 1 and 6 in group 2. Three reoperations were performed in group 1 and 6 in group 2. No statistically significant differences were found in the number of redislocations and reoperations (p = 50 and p = 0.48, respectively).

Introducción. La patología de los tendones peroneos es una causa frecuente de dolor posterolateral de tobillo. En los últimos años, la incidencia y el conocimiento de esta patología y de su tratamiento están en auge gracias al desarrollo de las técnicas tendoscópicas. Objetivo. Describir y evaluar el estado actual y las indicaciones de la tendoscopia en la patología de los tendones peroneos. Material y método. Desde junio de 2010 hasta julio de 2011 se realizaron 23 tendoscopias en pacientes con dolor retrofibular persistente. Encontramos 12 casos de rotura del peroneus brevis, 6 del peroneus longus, 3 casos de tenosinovitis y 2 casos de luxación, uno de ellos con una luxación intravaina y otra extravaina. De los 23 pacientes, 12 presentaban además otra lesión asociada: 4 lesiones osteocondrales de astrágalo, 3 inestabilidades anterolaterales de tobillo y 7 casos de pinzamiento de partes blandas. Discusión. Las 3 indicaciones principales de esta técnica son las tenosinovitis, las roturas tendinosas y la luxación de los tendones. Es un procedimiento técnicamente exigente, que requiere una amplia experiencia en el tratamiento artroscópico de pequeñas articulaciones y puede ser especialmente complejo en los casos de tenosinovitis extensa o amplias roturas tendinosas, pero muy útil para la evaluación y tratamiento de dicha patología. Conclusiones. La tendoscopia es un procedimiento de gran utilidad en el abordaje de la patología de los tendones peroneos, con baja morbilidad y excelentes resultados funcionales(AU)

MATERIAL AND METHODS: From June 2010 to July 2011, twenty three patients with retrofibular pain were treated with peroneal tendoscopy. We founded twelve peroneal brevis tendon tears, six peroneal longus tendon tears, three cases of tenosynovitis and two cases of luxation, one patient with an intrasheath subluxation and another one of extrasheath.
